Team Registration Form

Registering your team for the Team Up for The Children's Room  soccer tournament is easy!

Registration is due by June 6th, 2011.

1. Complete a Team Roster Form.

Please print and complete ONE Registration Roster Form for the entire team.

The minimum number of players per team is 6, including the goalie.

The maximum number of players per team is 12, including the goalie.

All players must be over the age of 18.  Proof of birthdate is required on game day in order to play.

2. Include Registration Fee Check.

Regular Registration: $400.00 due by June 6th, 2011. 

Please submit ONE check payable to: The Children’s Room.
